Description: The GUIA Case Associate works with patients with outside medical or social service referrals and helps direct them to outside resources, under the direction of the Clinical Services Manager. Project-specific opportunities may be available. Hours are flexible, standard shift is 3-5 hours with max 15 hours per week.

Duties: 

- Greets walk in patients to the clinic to address resource coordination 

- Obtains consent and availability from clinic patients with outside referrals 

- Request patient records for continuity of care 

- Ensure all tasks relating to a patient’s medical care is documented in the Electronic Medical Record 

- Promotes health literacy and suggests improvement for resource coordination

Qualifications:

- Must have accepted a Federal Work-Study award 2024-2025 academic year 

- Oral and written proficiency in English and Spanish 

- Proficiency in Microsoft Office 365 and Google Suite software 

- Strong professional written communication skills 

- Knowledge of clinical and healthcare-related concepts 

- Commitment to working with underserved patient populations

- Interest in social work, public health, or similar field 

- Previous experience in case management
